Family Practice - Without OB Physician

A health services network in Rockland County, New York is currently seeking a licensed Nurse Practitioner to provide Primary Care to their local LGBTQ+ community.

About the Opportunity:

Schedule: Monday to Friday
Specialty: Primary Care

Responsibilities:

Provide direct care and consultations to patients
Serve as a consistent Care Coordinator through the continuum of care
Assess the physical, psychological, spiritual, and social needs of the patient
Direct patients to the next appropriate service and track their progress
Assure that all patients receive appropriate evaluation, diagnosis, treatment, medical screening, and medical evaluation whenever indicated
Perform other duties, as needed

Qualifications:

NYS Nurse Practitioner (NP) license

Desired Skills:

Experience treating the LGBTQ community
