Key Responsibilities

As an Internal Medicine NP/PA, you will:

• Conduct thorough patient assessments by interviewing residents, performing physical exams, and reviewing medical histories to form comprehensive health profiles.
• Develop and implement individualized patient management plans that address both immediate health concerns and long-term wellness goals.
• Collaborate effectively with physicians and multidisciplinary teams to ensure continuity of care.
• Monitor patient progress and modify care plans as necessary based on individual responses to treatment.
• Educate patients and their families about health management and preventive care strategies.
• Document patient encounters and maintain accurate medical records in compliance with facility protocols and state regulations.
• Participate in patient care team meetings to discuss cases and strategies for improving resident health.

Patient Population / Client Focus

This role will primarily focus on the elderly population residing in the Assisted Living Facility, many of whom may have chronic ailments, complex medical histories, or require palliative care. You will be required to understand the unique needs of this demographic, emphasizing holistic care and compassion in every interaction to enhance their quality of life.

Qualifications

To be considered for this role, candidates must meet the following qualifications:

• Graduation from an accredited Nurse Practitioner or Physician Assistant program.
• A minimum of two years of clinical experience in an ambulatory care setting, specifically in internal medicine or geriatric care.
• Current state licensure as a Nurse Practitioner or Physician Assistant in New York.
• Successful completion of the Physician Assistant National Certifying Examination is highly preferred.

Preferred Experience

While not required, additional experience in an Assisted Living or similar setting will enhance your application. Proficiency in electronic health record systems is also desirable as it supports effective documentation and streamlined communication with the healthcare team.

Compensation and Benefits
