Even though the role is remote, collaboration with multidisciplinary teams and direct engagement with clients in various settings, including our Hospice Center, remains vital. Key Responsibilities\n

• Conduct individual assessments for clients facing significant mental health challenges, employing evidence-based screening and evaluation tools to develop tailored care plans.

• Engage in therapeutic interventions, offering individual, group, and family therapy sessions aimed at promoting healing and recovery.

• Collaborate closely with a team of healthcare professionals, facilitating communication and coordination of care among clients, families, and other service providers.
\n
• Develop, implement, and review integrated treatment plans to ensure that clients have access to necessary community services and resources.
\n
• Assist with crisis intervention efforts, providing immediate support during acute mental health emergencies.
\n
• Monitor clients' progress, adjusting treatment plans as needed while ensuring compliance with admission, discharge, and continued stay criteria.

• Participate in quality improvement initiatives and committees aimed at enhancing service delivery and client outcomes.

• Maintain comprehensive and timely documentation of client interactions and treatment progress to ensure proper data submission and reporting.

Qualifications\n

Compensation and Benefits\n
