Key Responsibilities\n

• Conduct comprehensive patient intakes focusing on diagnostic evaluations and functional assessments, followed by making informed recommendations to primary care providers regarding treatment goals and plans.

• Provide therapeutic interventions, utilizing evidence-based techniques, to address mental health challenges while monitoring patients' progress through documented treatment charts.

• Collaboratively engage with primary care providers by assisting in the recognition, treatment, and management of psychosocial issues within the context of overall health care.

• Identify and manage 'at risk' patients, collaborating with the healthcare team to develop preventive strategies and relapse prevention plans.

• Initiate follow-ups with patients to assess their mental health progress, adjusting treatment plans as needed based on their feedback and changing conditions.

• Facilitate knowledge transfer of self-management skills to patients, empowering them to take an active role in their treatment journey.

• Maintain professional ethics as mandated by the Code of Ethical Conduct for Clinical Professionals, handling sensitive information with the utmost integrity.

• Contribute to interdepartmental meetings providing insights on behavioral health needs and program evaluation.

• Support patients in adhering to medical treatment recommendations from primary care providers, including strategies for managing medication side effects.

Qualifications\n

Compensation and Benefits\n

The compensation for this position ranges from $87,000 to $105,000 annually, depending on experience and qualifications.
