Key Responsibilities\n

• Support primary care providers by identifying, treating, and managing mental health and psychosocial issues, thereby acting as a vital member of the healthcare team.

• Conduct comprehensive patient intakes focusing on diagnostic assessments, and make informed treatment recommendations to the primary care team based on these evaluations.

• Deliver brief, focused therapeutic interventions tailored to the unique needs of patients seeking mental health services.

• Advise primary care providers on appropriate referrals to specialty mental health services when necessary to ensure optimum patient care.

• Initiate and track follow-up consultations to monitor patient progress and adapt treatment plans as needed, enhancing the quality of recovery.

• Develop and implement relapse prevention strategies for patients to promote stable functioning and support mental health maintenance.

• Identify at-risk patients and collaborate with the healthcare team to create comprehensive plans to mitigate worsening conditions.

• Coordinate and oversee health services related to behavioral health, ensuring communication with other providers, both within and outside the clinic, with patient consent.

• Document patient progress and diagnostic information accurately in treatment charts to maintain updated clinical records.

• Keep primary care providers informed about patients’ needs and progress, working collaboratively to optimize treatment plans.

• Facilitate lifestyle changes for patients at risk of physical disorders, empowering them to make healthier choices.

• Provide patients with educational resources and self-management skills to encourage active participation in their treatment journey.

• Consult with clinic management and team members regarding behavioral services and potential areas for program evaluation and improvement.

• Assist patients in adhering to prescribed medical treatments, including offering strategies to handle medication side effects.

• Uphold the Code of Ethical Conduct for Clinical Professionals, ensuring integrity and professionalism in all interactions.

Qualifications\n

• A Master's Degree in Clinical Counseling or a related field.

• A current LPC (Licensed Professional Counselor) license is required.

Compensation and Benefits\n

The salary range for this position is USD 87,000 – 105,000 per year, commensurate with experience and qualifications.
