Key Responsibilities\n

• Conduct comprehensive and ongoing assessments to effectively identify and address the mental health needs of patients.

• Facilitate communication among supervisors, direct care team members, families, and relevant external collaborators to ensure cohesive support for each patient’s progress.

• Create and manage a tentative schedule for appointments, home visits, and progress monitoring activities, ensuring thorough follow-up on patient care.

• Oversee coordination for home visits that include in-home observations, detailed chart reviews, and interviews, providing necessary data for informed care.

• Implement and evaluate patient program plans, ensuring adherence to established methodologies and professional standards.
\n
• Engage actively in Interdisciplinary Team Meetings to collaborate with healthcare providers on patient care strategies.
\n
• Document monthly and quarterly progress notes, maintaining accurate and updated records of each patient’s journey.
\n
• Organize and lead training sessions to ensure all staff are competent in the implementation of program plans and documentation practices.

• Conduct follow-up interviews and continuous home visits for sustained support and engagement with patients.

• Serve as a liaison to the broader mental health community, fostering cooperative relationships that aid in the development of patient goals.

Patient Population / Client Focus\n

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field is required.

• Must hold an active and unrestricted licensure as a Professional Counselor (LPC) within the state of Texas.

• A minimum of two years of direct counseling experience is essential, preferably within a clinical setting.

• Preferred qualifications include specialization in family therapy, couples counseling, substance misuse therapy, or trauma-informed care.
