Key Responsibilities\n

• Conduct thorough assessments on new and ongoing patients to tailor counseling strategies that align with their specific needs.

• Effectively communicate patient progress to supervisors, as well as family members and other stakeholders involved in patient care.
\n
• Develop a well-organized schedule that includes appointments, home visits, and observations aimed at monitoring patient progress and ensuring ongoing support.
\n
• Coordinate home visits to facilitate comprehensive evaluations through observations, chart reviews, and staff interviews.
\n
• Ensure the successful implementation and execution of individualized care plans, adjusting strategies as necessary to optimize patient outcomes.
\n
• Actively participate in interdisciplinary team meetings, contributing to discussions on treatment strategies and patient care.
\n
• Accurately document monthly and quarterly progress notes, ensuring compliance with legal and ethical standards in mental health practice.
\n
• Organize and lead training sessions for staff to ensure proficiency in program implementation and adherence to documentation protocols.
\n
• Provide ongoing support through follow-up interviews and home visits, adapting to changing patient needs.
\n
• Maintain comprehensive records that ensure transparency and compliance with all applicable regulations.

• Act as a liaison with external mental health resources and the public to support the development of mental health initiatives within the community.

• Assist in the creation of procedural guidelines and training materials aimed at enhancing program delivery and effectiveness.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely-related field is required.

• Active and unrestricted licensure as a Licensed Professional Counselor (LPC) in Texas.

• A minimum of 2 years of clinical experience in an appropriate counseling setting is essential.

• Specialized knowledge or certifications in family therapy, couples therapy, substance misuse, or trauma-informed care will be highly regarded.
