Key Responsibilities\n

• Perform thorough and ongoing assessments to identify the unique needs of each patient, ensuring tailored treatment plans.

• Maintain open lines of communication among supervisors, direct care staff, families, and other significant individuals regarding patient progress.

• Prepare and provide a schedule of appointments, home visits, and observational assessments to supervisors for tracking therapeutic advancement.

• Coordinate home visits to conduct necessary in-home observations, chart reviews, and staff interviews as part of a holistic assessment process.
\n
• Implement and evaluate the efficacy of the program plans, ensuring that all interventions are being executed appropriately.
\n
• Actively participate in Interdisciplinary Team Meetings and other relevant consultations as needed to promote collaborative care.
\n
• Document patient progress through comprehensive monthly and quarterly notes, ensuring accuracy and availability of essential information.
\n
• Organize and facilitate training for team members on the implementation of program protocols and documentation practices.
\n
• Conduct follow-up interviews and provide continued support through home visits to monitor individuals' progress and needs.
\n
• Serve as a liaison with the public and the mental health community, aiding in the development of support networks for clients.

• Assist in developing training materials and procedures that enhance program effectiveness for the populations we serve.

Patient Population / Client Focus\n

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Must hold active and unrestricted licensure as a Professional Counselor (LPC) in Pennsylvania.
