Key Responsibilities\n

• Conduct comprehensive assessments to determine individual patient needs, ensuring that each client receives personalized care.
\n
• Engage in effective communication with supervisors, fellow healthcare professionals, family members, and guardians regarding client progress and necessary adjustments to treatment plans.
\n
• Develop and maintain a tentative schedule of appointments, home visits, and other essential observations to monitor and support client progress.
\n
• Organize and coordinate home and telehealth visits to fulfill treatment requirements, performing chart reviews and interviews as needed.
\n
• Oversee the implementation of treatment plans, ensuring they are executed efficiently and effectively in accordance with best practices.
\n
• Participate actively in Interdisciplinary Team meetings to discuss patient progress and collaborate on care strategies.
\n
• Document and complete monthly and quarterly progress notes, maintaining patient confidentiality and accuracy of records.
\n
• Lead training sessions to enhance staff competencies related to program implementation and documentation practices.
\n
• Provide ongoing support through follow-up interviews and assessments to address evolving client needs.
\n
• Act as a liaison between the mental health community and public stakeholders, fostering collaboration and resource sharing.
\n
• Contribute to the creation and refinement of operational procedures and training materials to enhance program efficacy.

Patient Population / Client Focus\n

You will work with a diverse client base, including both adults and youth facing a variety of mental health challenges. As a counselor, you will play a crucial role in addressing issues such as anxiety, depression, trauma, and substance use, providing both support and intervention tailored to each client's unique circumstances.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related discipline is required.
\n
• An active and unrestricted license as a Professional Counselor (LPC) in California.
\n
• A minimum of two years of counseling experience, preferably in a clinical setting.
\n
• Demonstrated specialization in areas such as family therapy, trauma-informed care, or substance use counseling is highly desirable.

• Strong knowledge of legal and ethical standards governing mental health practices.
\n
• Exceptional interpersonal skills with the ability to build rapport and trust with a diverse range of clientele.

• Proficient in collaborating effectively within a multidisciplinary healthcare team.
\n
• Experience utilizing Electronic Medical Records (EMR) software and related technology.
\n

Preferred Experience\n

While not required, prior experience working within telehealth services or familiarity with crisis intervention strategies can be beneficial. Additional competencies in therapy techniques tailored to adolescents or families will also be considered an asset.
