Key Responsibilities\n

• Conduct thorough assessments to understand the ongoing mental health needs of clients, ensuring tailored care strategies.

• Serve as the primary contact for communication between clients, supervisors, caregivers, and other essential stakeholders, providing updates on client progress.

• Develop tentative schedules outlining appointments, home visits, and necessary observations to monitor and evaluate care delivery effectively.

• Facilitate home visits to conduct in-home observations and gather relevant data through chart reviews and interviews.

• Implement, monitor, and adapt program plans, ensuring all interventions are effective and client's needs are being met.

• Participate actively in interdisciplinary team meetings to enhance client care and improve service delivery.

• Document monthly and quarterly progress notes to maintain accurate client records and treatment strategies.

• Organize and lead training sessions to ensure that all staff are proficient in their roles and knowledgeable about program implementation and documentation.

• Provide ongoing support through follow-up sessions, home visits, and check-ins to ensure lasting success for clients.

• Actively engage with the broader mental health community to foster relationships that support developmental goals.

Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related field from an accredited institution.

• Active and unrestricted licensure as a Licensed Professional Counselor (LPC) in the state of Texas.

• A minimum of 2 years’ experience in a clinical counseling setting, showcasing your ability to provide effective mental health care.

• Specialization or experience in family therapy, couples therapy, trauma-informed care, or substance misuse treatment is highly regarded.
