Key Responsibilities\n

• Conduct ongoing assessments to develop individualized treatment plans tailored to the specific needs of each patient.

• Coordinate home visits and virtual follow-ups to ensure comprehensive care and support, including chart reviews and staff interviews to gauge client advancement.

• Continuously evaluate the effectiveness of implemented treatment strategies and revise as necessary, ensuring alignment with best practices in mental health care.

• Participate actively in Interdisciplinary Team Meetings to discuss patient progress and collaborate on treatment strategies.

• Document monthly and quarterly progress notes to provide updates on client status, engaging in detailed record-keeping for accountability.

• Organize and lead training initiatives to certify staff in program implementations and documentation practices, ensuring therapeutic standards are met.

• Act as a liaison with the broader mental health community, fostering partnerships aimed at enhancing community support systems.

Qualifications\n

Compensation and Benefits\n

We offer a competitive salary ranging from $90,000 to $100,000 annually, commensurate with experience and caseload.
