Key Responsibilities\n

• Conduct thorough, ongoing mental health assessments and evaluations to tailor treatment plans effectively to meet the unique needs of each client.

• Actively communicate with supervisors, family members, and other stakeholders to discuss patient progress, ensuring a holistic approach to care.

• Coordinate home visits and logistical arrangements to observe clients in their daily environments, facilitating comprehensive evaluations.

• Implement and monitor the treatment plan, evaluating its effectiveness and making adjustments as necessary.

• Participate in interdisciplinary team meetings to collaborate with healthcare professionals and ensure comprehensive care delivery.

• Document patient progress through detailed monthly and quarterly reports, ensuring compliance with legal and ethical standards.

Qualifications\n
