Key Responsibilities\n

• Conduct thorough assessments to identify the specific mental health needs of each patient, using evidence-based practices as a guide.

• Actively communicate with patients, their families, and other stakeholders to share insights regarding treatment progress and adapt care plans accordingly.

• Develop a regular schedule of therapy sessions, home visits, and necessary observations to ensure continuity of care and monitor client progress.

• Coordinate home visits in compliance with clinical standards, encompassing chart reviews, data analysis, and interviews to inform treatment efficacy.

• Ensure that all aspects of the treatment program are delivered effectively and are continuously adapted to meet patient needs.

• Participate in interdisciplinary team meetings, sharing insights and collaborating on patient care strategies.
\n
• Document detailed progress notes monthly and quarterly to provide a clear record of client advancement and interventions.
\n
• Organize and conduct training sessions to uphold high standards of program implementation among staff.
\n
• Provide ongoing follow-up and support through home visits and consultations to ensure that each individual's needs are met.

• Maintain meticulous client records, ensuring that they are readily accessible for appropriate stakeholders.

• Serve as a liaison between the public and the mental health community, facilitating collaboration and resource sharing towards developmental goals.

• Assist in creating and refining training materials and procedures that enhance program delivery and client outcomes.

Qualifications\n
