Key Responsibilities\n

• Conduct thorough assessments of clients' mental health needs and develop personalized treatment plans to address their unique challenges.

• Maintain open lines of communication with supervisors, direct care staff, families, and collateral contacts to discuss the client’s progress and adapt care as needed.

• Provide a structured schedule of appointments, home visits, and client observations to monitor advancements in therapy.

• Coordinate in-home observations and chart reviews to ensure comprehensive assessments and the quality of care.

• Implement and oversee treatment plans, providing feedback, and making adjustments to ensure effectiveness.

• Participate actively in Interdisciplinary Team Meetings to align client care strategies with broader organizational goals.

• Document monthly and quarterly progress through detailed notes, ensuring all information is accessible to improve client care.

• Organize and facilitate training for staff to ensure competence in program implementation and proper documentation procedures.

• Conduct follow-up interviews and home visits post-treatment to assess continued support for clients.

• Act as a liaison within the mental health community to promote program initiatives and gather community resources.

• Assist in creating and improving training materials and procedures that support effective client interventions.

Qualifications\n
