Key Responsibilities\n

• Conduct thorough and ongoing assessments of patients to accurately determine their mental health needs and develop tailored treatment plans.

• Facilitate regular communication with supervisors, direct care staff, family members, and other stakeholders to ensure comprehensive support for patients and track their progress.

• Develop and deliver a tentative schedule of appointments, home visits, and observations to monitor client advancement and assess the efficacy of treatment plans.

• Coordinate home visits to collect data via observations and chart reviews that inform treatment strategies and align with program goals.

• Ensure that all program plans are effectively implemented and adhered to while maintaining high standards of care throughout the treatment process.

• Actively participate in Interdisciplinary Team Meetings, contributing insights from assessments and adapting strategies in collaboration with healthcare professionals.

• Maintain regular documentation of monthly and quarterly progress notes to monitor developments and changes in patient conditions.
\n
• Lead training sessions aimed at certifying staff on program implementation, best practices, and compliance with documentation procedures.
\n
• Conduct follow-up interviews and home visits to provide ongoing support and adjust treatment plans collaboratively with clients.

• Serve as a liaison between the clinic, public agencies, and the broader mental health community to assist in achieving development goals for each patient.

• Assist in developing procedures and training materials to enhance program delivery and outcomes for individuals receiving care.

Qualifications\n
