Key Responsibilities\n

• Collaborate with primary care practitioners to identify, treat, and manage mental health and psychosocial issues, thereby enhancing the efficacy of the overall care team.

• Guide primary care teams in deciding which patients may benefit from specialty mental health services or referrals, paving the way for comprehensive treatment plans.

• Help patients develop and implement relapse prevention strategies, empowering them to manage their mental health proactively.

• Identify patients at risk of deteriorating health and participate in designing prevention programs to address their specific needs.

• Identify and document potential behavioral changes and recommend lifestyle adjustments to patients with mental health challenges.

Qualifications\n
