Key Responsibilities\n

• Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial issues as an integral member of the care team.

• Conduct thorough patient intakes that include diagnostic and functional evaluations, making informed recommendations for treatment goals and plans.

• Deliver focused intervention strategies for patients requiring mental health services, often in high-pressure situations.

• Advise primary care providers on the appropriate care setting for patients, determining whether they should continue receiving care within the primary care setting or be referred to specialized mental health facilities.

• Initiate follow-ups to monitor patient progress and adjust treatment strategies as necessary, ensuring continuous improvement in care.

• Develop relapse prevention plans as required, aiding in the mental stability of patients.
\n
• Identify "at risk" patients and collaborate on preventive strategies to avert deterioration of their conditions.
\n
• Coordinate health service delivery related to behavioral health care, linking with treatment providers inside and outside of the primary care environment with patient consent.
\n
• Maintain accurate and detailed clinical documentation of patient progress and diagnostic information.
\n
• Keep primary care providers fully updated on patient needs and progress, contributing to the development of personalized treatment plans.

• Promote behavioral changes in patients at risk for physical disorders, assisting them in making healthier lifestyle choices.
\n
• Equip patients with self-management skills and educational resources to encourage active participation in their treatment and recovery journey.
\n
• Consult on behavioral services with clinic management and team members, providing insights into outcome evaluations and program enhancements.
\n
• Assist patients in adhering to medical treatments initiated by primary care providers, including strategies to cope with side effects.

• Adhere strictly to the Code of Ethical Conduct for Clinical Professionals while executing other assigned duties.

Qualifications\n

• A Master's Degree in Clinical Counseling from an accredited institution.

• A valid, current LPC (Licensed Professional Counselor) license.

• Proficiency in diagnosing mental health disorders using DSM-IV criteria.

• Preferred bilingual proficiency in English and Spanish, although not mandatory.

• Familiarity with community resources and the ability to navigate them effectively.

• Excellent clinical record-keeping and report preparation skills.

Preferred Experience\n

Compensation and Benefits\n

The salary for this role ranges from approximately USD 87,000 to USD 105,000 annually, commensurate with experience and qualifications. Along with competitive compensation, selected candidates will also be eligible for a sign-on bonus.
