RN Intensive Community Care Manager — Home-Based Leader

ChenMed

ChenMed in Virginia is seeking an Intensive Community Care Manager (RN) to lead the Complex Care Team for high-risk patients, coordinating care plans with PCPs and clinicians to prevent unnecessary hospital visits and keep patients safely at home. You will perform home visits, assess needs, develop comprehensive care plans, provide supervision to team members, and ensure smooth transitions of care.
