Care Coordinator

We are seeking a Care Coordinator for the Children's Long Term Support (CLTS) Waiver Program to support children with developmental, physical, or severe emotional disabilities. In This Role, You Will Conduct functional assessments and develop individualized service plans with families and providers Authorize and coordinate services in alignment with plans Facilitate family-centered team meetings and maintain accurate documentation Collaborate with service teams and supervisors to determine interventions Testify at legal proceedings when necessary Promote client independence, provide trauma-informed, family-centered services using evidence-based practices, and participate in staff development Qualifications Include Bachelor's degree in a health or human services field At least 1 year of experience working with children in the target population Strong communication, documentation, organizational skills, and proficiency with electronic health records Valid driver’s license and reliable transportation Bilingual fluency in Spanish is preferred but not required The Position Offers Medical, dental, and vision insurance Flexible spending accounts and mileage reimbursement Paid time off, holidays, and a 403(b) retirement plan Employee assistance and recognition programs Remote work options (up to two days per week) Work Environment Community-based with daily travel and occasional overnight stays Moderate outdoor exposure and ability to handle crisis situations #J-18808-Ljbffr Seacrest Behavioral Health
