About This Role The Transfer Center RN is responsible for facilitating communication between sending and receiving facilities, utilizing clinical expertise and critical thinking to ensure proper patient transfers. This role is ideal for experienced RNs who thrive in a fast-paced environment and excel in collaboration. Highlights Location: Hackensack, NJ Facility: Hackensack Meridian Health Schedule: Per Diem, Day shift What You’ll Do Facilitate communication between sending and receiving facilities. Document a record of the encounter using electronic health records. Answer telephones in accordance with established policies and procedures. Coordinate communication within the receiving facility. Accurately record details of all communication related to transfer requests. Collaborate with capacity management, nursing, and physician/provider representatives. Maintain awareness of bed availability on all patient care units. Handle high-pressure situations and establish priorities for effective work completion. Collaborate with Emergency Medical Services (EMS) for timely transportation. Initiate registration activities and complete registration as requested.
