Role highlights: Deliver high-quality, patient-focused clinical pharmacy services Undertake medicines reconciliation, reuse checks, and ordering Work closely with multidisciplinary teams to optimise medicines use Support specialist areas includingperinatal mental health(Mother& Baby Unit),CAMHsinpatient services,low secure forensic units& MHIST (mental health intensive support team) Contribute to service development, audit,teachingand quality improvement Opportunities to develop clinicalexpertiseand leadership skills To provide, develop and promote a responsive, patient focused, highly specialised clinical pharmacy service to patients and staff within Cheshire and Wirral Partnership NHS Foundation Trust. To deliver specialist information and expert advice to all members of the multidisciplinary team to maximise patient pharmaceutical care and ensure that medicines are prescribed, administered and supplied in a safe, effective and economic manner and to reduce harm caused by medication errors. To provide direct supervision and training to clinical pharmacy staff, develop and provide training regarding the use of medicines and medicines management principles to clinical staff, non-clinical staff, service users and carers. To support the Head of Clinical Pharmacy Services- Bed Based & Place Based in the delivery of all aspects of the Pharmacy Service Cheshire and Wirral Partnership (CWP) provides health and care services for a population of over one million people, including mental health, learning disability, community physical health and all-age disability care, as well as the provision of three GP surgeries in Cheshire. We employ around 4,500 staff across 73 sites and have services across Wirral and Cheshire, as well as Trafford, Warrington, Bolton, Halton, Liverpool and Stockport.
