Work Environment\n

This position offers a remote work setting, with the flexibility needed to accommodate client visits and ongoing care requirements. Although the role is primarily conducted from home, you will be integrated into a team of interdisciplinary professionals, including nurses and healthcare providers, who work cohesively to ensure clients receive comprehensive support. Standard work hours are Monday through Friday, from 8:00 AM to 4:30 PM, with potential for evening or weekend hours based on client needs.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors’ psychosocial needs, taking into account their emotional, mental, and social circumstances.

• Collaborate with clients and their families to create personalized care plans aimed at addressing their unique goals and preferences.

• Provide counseling and emotional support, aiding clients through significant life transitions and complexities.

• Advocate vigorously for clients’ rights, helping them access necessary community resources and services.

• Work closely with interdisciplinary teams to coordinate care and improve outcomes for seniors.

• Conduct regular client visits to monitor well-being, assess living conditions, and tackle any safety concerns.

• Facilitate communication among family members involved in caregiving to optimize care strategies.

• Educate clients and families about available community supports, enhancing their social engagement and well-being.

• Maintain detailed and accurate documentation of assessments, care plans, and other relevant client interactions in accordance with ethical standards.

Qualifications\n

• Master's degree in Social Work (MSW) from an accredited institution.

• Active and valid state licensure as a Licensed Master Social Worker (LMSW).

• A minimum of two years of experience in a social work capacity, preferably in a community health or geriatric setting.

Compensation and Benefits\n
