Key Responsibilities\n

As a Licensed Clinical Social Worker, you will be instrumental in providing clinical behavioral health services. Your daily responsibilities will include:

• Assisting mental health consumers in navigating the complexities of the physical and behavioral healthcare systems, ensuring they receive integrated care.

• Educating patients and families about available resources and services, promoting engagement in preventive health measures, wellness, and overall healthy lifestyle choices.

• Participating in regular multidisciplinary treatment team meetings, contributing insights and collaborating with healthcare professionals to develop individualized care plans.

• Promptly reporting any crisis or unusual incidents to the Medical Director, and immediately escalating psychiatric and medical crises to the Chief Medical Officer (CMO).
\n
• Maintaining comprehensive and timely documentation within clinical records, adhering to all relevant policies and standards.
\n
• Engaging in Quality Improvement initiatives, staying informed through meetings and training to continually enhance service delivery.
\n
• Upholding compliance with all organizational policies, working seamlessly within the integrated Primary Care Behavioral Health model.
\n
• Managing evidence-based screening tools to accurately assess the healthcare needs of individuals, with a focus on reporting results effectively.
\n
• Working collaboratively with the Center of Excellence Team to support patients struggling with Opioid Use Disorder.
\n

Patient Population / Client Focus\n

This role targets a diverse patient population, particularly individuals receiving hospice care who may be facing complex physical and mental health challenges. You will engage with patients and families during emotionally sensitive times, providing the support and resources necessary to navigate their healthcare journey.

Qualifications\n

The successful candidate will possess:

• A Master’s degree in Social Work from an accredited institution.

• A valid Licensed Clinical Social Worker (LCSW) credential.
\n
• At least 2-3 years of relevant experience in clinical social work, particularly within healthcare settings.

• The ability to collaborate effectively with healthcare professionals, social service agencies, and clients.
\n
• Flexibility to travel to various CHDC locations for meetings, training sessions, and outreach activities as needed.

Preferred Experience\n

Preference will be given to candidates who have:
