Key Responsibilities\n

• Conduct thorough psychosocial assessments through interviews, environmental observations, and comprehensive reviews of medical records to identify factors affecting recovery, addressing emotional, social, and environmental influences.

• Collaborate closely with the rehabilitation team, including physicians, occupational and physical therapists, to formulate individualized treatment plans that align with patients' psychosocial needs.

• Guide patients in accessing vital community resources, including support groups, vocational rehabilitation, and housing assistance, ensuring they have the tools necessary for successful reintegration.

• Deliver targeted education and guidance for families regarding community resources and available post-rehabilitation services to ensure continuity of care.

• Participate actively in discharge planning, helping to facilitate seamless transitions to home or other care settings.

Compensation and Benefits\n
