Key Responsibilities\n

• Conduct thorough psychosocial assessments through interviews, observations, and comprehensive review of patient medical records to evaluate various emotional, social, and environmental factors that may affect recovery.

• Provide empathetic counseling and emotional support to patients and their families, assisting them in navigating adjustment issues and enhancing their emotional well-being during critical rehabilitation phases.

• Work collaboratively with the rehabilitation team to develop tailored treatment plans, ensuring that psychosocial aspects of patient care are addressed effectively.

• Assist patients in accessing valuable community resources, such as support groups, vocational rehabilitation services, and housing assistance, fostering a holistic recovery approach.

• Educate patients and their families on available community resources and post-rehabilitation services to ensure continuation of care and support.

• Participate actively in discharge planning, ensuring seamless transitions to home care or other appropriate care settings.

Patient Population / Client Focus\n

The LCSW will primarily serve patients with a wide range of conditions, including but not limited to, recovery from strokes, spinal cord injuries, orthopedic surgeries, and those dealing with neurological disorders. This role recognizes the importance of addressing both the psychological and social determinants of health in promoting successful rehabilitation outcomes and community reintegration.

Qualifications\n

• A Master's degree in Social Work from an accredited program.

Compensation and Benefits\n
