Assist individuals with transportation, daily activities, community participation, and achieving goals outlined in their Individual Support Plans and Behavior Support Plans. Promote independence, dignity, personal choice, inclusion, and the development of life skills. Maintain accurate documentation including medication administration records, daily logs, progress notes, incident reports, and other required records. Collaborate with coworkers, families, supervisors, and support teams to ensure high-quality services and compliance with agency and regulatory requirements.
