Key Responsibilities\n

• Collaborate with primary care providers to identify, address, and manage mental health and psychosocial issues for patients.

• Conduct thorough patient intakes, including diagnostic and functional evaluations, followed by actionable treatment recommendations for the primary care team.

• Implement brief, solution-focused interventions for clients in need of mental health support.
\n
• Assess and refer patients to specialty mental health facilities as needed, ensuring they receive optimal care tailored to their individual situations.

• Initiate follow-up contacts to monitor patient progress and adapt treatment plans as required.
\n
• Develop relapse prevention strategies and assist patients in maintaining stable functioning.

• Identify at-risk patients and create proactive plans to mitigate deterioration of conditions.

• Manage and document patient care, maintaining comprehensive records in accordance with established protocols.

• Educate patients about their health conditions, empowering them with self-management skills to enhance their recovery experiences.

• Consult with clinic management and team members to refine behavioral health services and evaluate program outcomes.

Qualifications\n

Compensation and Benefits\n

The annual salary for this position ranges from USD 87,000 to 105,000, commensurate with experience and qualifications.
