Key Responsibilities\n

Your day-to-day responsibilities will include, but are not limited to, the following:

• Conduct thorough assessments of clients' counseling needs, identifying key factors impacting their mental health and substance use patterns.

• Utilize the DSM (Diagnostic and Statistical Manual) criteria to perform accurate assessments for substance abuse diagnoses, determining the right level of care based on ASAM (American Society of Addiction Medicine) standards.

• Prepare individualized treatment plans incorporating substance abuse counseling methods tailored to the unique needs of each client.
\n
• Develop comprehensive discharge and continuum of care plans to facilitate smooth transitions for clients at various stages of recovery.
\n
• Document all counseling sessions, assessments, and outcomes in client clinical records to maintain compliance and continuity of care.
\n
• Manage a caseload of clients, ensuring all aspects of treatment planning, case management, and utilization reviews are documented and updated regularly.
\n
• Provide coordination of additional support services for clients and their families, including noting available resources in clinical records.
\n
• Engage with referral sources such as schools, legal entities, and employers to coordinate necessary services and interventions for clients.
\n
• Identify and implement immediate interventions for crisis-oriented issues, frequently collaborating with a multi-disciplinary team.

Patient Population / Client Focus\n

You will primarily engage with clients facing challenges related to substance use disorders as well as co-occurring mental health issues. Patients served may vary in age and background, necessitating a culturally competent approach to care that respects and responds to each individual's unique circumstances. Your expert guidance will play a pivotal role in supporting them toward recovery and improved quality of life.

Qualifications\n

The successful candidate will hold a Bachelor's Degree in Social Work (BSW) and possess a valid LCSW licensure. Moreover, strong candidates will demonstrate:
