Health care helps.For nearly 35 years, Fourth Street Clinic has provided essential health care services to individuals experiencing homelessness in the Salt Lake City area. Joining our team means more than taking a job-you're directly supporting some of our community's most vulnerable members and helping them navigate their healthcare journey.Position OverviewThe Medical Case Manager (MCM) is a key member of the Medically Vulnerable Population (MVP) team, providing patient-centered case management for unhoused and medically vulnerable individuals. The MCM conducts needs assessments, develops personalized care plans, coordinates medical and behavioral health services, and facilitates access to supportive resources. This role ensures continuity of care, addresses social determinants of health, and advocates for patients to achieve optimal health outcomes.Key ResponsibilitiesProvide medical case management for a designated caseload of 25-34 patientsConduct thorough needs assessments for medically vulnerable patientsDevelop and update individualized, patient-centered care plansCoordinate and facilitate delivery of prescribed medical and supportive servicesArrange transportation for patients to access services both within the clinic and with community partnersCollaborate with interdisciplinary teams and external service providers to ensure continuity of careMaintain accurate patient documentation and meet medical/data reporting requirementsStay current on clinic policies, procedures, systems, and electronic health recordsParticipate in weekly check-ins with the MVP Clinic ManagerSupport a healthy work environment and uphold FSC's values, fostering respect, trust, and collaborationRequired QualificationsHigh School Diploma or equivalent (Bachelor's degree in a healthcare-related field preferred)Experience in medical case management or related patient support functions (preferred: 2+ years in ambulatory care)Strong attention to detail and commitment to accuracy in all workAbility to work collaboratively and demonstrate a caring, compassionate approach to patient carePreferred QualificationsBilingual in English and SpanishExperience working with medically vulnerable or unhoused populationsCompensation and Benefits100% employer-paid individual medical insuranceAffordable dental & vision coverageHealth Savings Account (HSA) with employer matchCompany-paid Life & Long-Term Disability insurance401(k) with 3% employer contribution plus dollar-for-dollar match up to 2%Generous paid time-off: 3+ weeks/year for full-time employees13 paid holidays, plus 2 personal holidays and 2 wellness daysTuition reimbursement and Employee Assistance Program (EAP)Why Join Fourth Street Clinic?Fourth Street Clinic improves health outcomes for Utahns experiencing homelessness through high-quality, integrated, trauma-informed care.
