As a Licensed Clinical Social Worker at CHDC, you will be entrusted with a diverse array of responsibilities that significantly contribute to patient outcomes and organizational goals. Key responsibilities include:

• Providing comprehensive clinical behavioral health services that align with CHDC's mission and quality standards, aiming to enhance patient health outcomes and satisfaction.

• Assisting mental health consumers in understanding and navigating the complex web of physical and behavioral healthcare systems to ensure integrated healthcare delivery.

• Educating clients about available resources and services to foster engagement in disease prevention, wellness initiatives, and the promotion of a healthier lifestyle.

• Collaborating with multi-disciplinary treatment teams during regular meetings to devise and adjust treatment plans based on patient progress.

• Timely and accurate reporting of any crises or unusual incidents, ensuring that psychiatric and medical emergencies are escalated immediately to the appropriate director or Chief Medical Officer.

• Maintaining meticulous clinical documentation within individual patient records in compliance with legal and organizational standards.
\n
• Engaging in CHDC’s Quality Improvement program by actively participating in assigned training sessions and committee meetings.

• Utilizing evidence-based screening tools to effectively assess the healthcare needs of individuals and reporting outcomes to inform care strategies.

• Working closely with patients experiencing Opioid Use Disorder, collaborating with our Center of Excellence Team to facilitate comprehensive treatment and support.

Patient Population / Client Focus\n

Your primary focus will be on patients requiring detox services and those dealing with substance use and mental health disorders. You will play a vital role in offering behavioral health support tailored to the unique challenges faced by this population. Through your expertise, you will help facilitate smoother transitions into and out of detox services, ultimately aiming for the best possible recovery outcomes.

Qualifications\n

To be considered for this role, candidates must meet the following qualifications:

• A Master’s degree in social work and hold a valid Licensed Clinical Social Worker (LCSW) license.

• A minimum of 2 to 3 years of experience in clinical social work, preferably within a medical or community health setting.
