Key Responsibilities

As a Physical Therapist with Confluent Rehabilitation, your responsibilities will include:

• Designing personalized physical therapy treatment plans that focus on alleviating pain, enhancing mobility, and preventing disability according to the physician's orders.
• Conducting thorough evaluations of clients' functional abilities and home environments to create effective and customized rehabilitation strategies.
• Administering therapeutic exercises, manual therapy, massage, and other interventions designed to improve strength and functionality.
• Supervising physical therapy assistants, ensuring that they adhere to the established care plans and deliver high-quality services.
• Documenting patient progress and modifying care plans based on patient feedback and clinical observations.
• Participating in case conferences with interdisciplinary teams to refine and discuss best practices in patient care.
• Educating clients and their families about their physical therapy program and the proper use of therapeutic devices.

Patient Population / Client Focus

You will work with a diverse patient population, primarily consisting of individuals recovering from surgeries, those managing chronic conditions, and elderly clients seeking to maintain their independence through rehabilitation. By understanding each client's unique needs and challenges, you will play a vital role in facilitating their progress and ensuring they receive the compassionate and effective care they deserve.

Qualifications

The successful candidate will possess the following qualifications:

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• A valid PT license, in good standing, in the State of New Jersey.
• Current CPR certification and a valid driver's license with automobile insurance.
• A commitment to high-quality patient-centered care and a willingness to embrace innovative therapeutic approaches.
• Excellent interpersonal skills for effective communication with clients, families, and interdisciplinary team members.

Preferred Experience

Compensation and Benefits
