Key Responsibilities

• Conduct thorough evaluations of patients, including a detailed review of their medical history and specific assessments, to tailor effective treatment plans.
• Clearly communicate evaluation findings and treatment goals to patients and their families, partnering with them in the goal-setting process to encourage active participation in their recovery.
• Implement individualized treatment protocols based on evaluation insights, aligning with the patient's current conditions and adapting to changing healthcare environments.
• Collaborate with a multidisciplinary team of healthcare providers to ensure comprehensive patient care, documenting any significant changes in the patient's condition and adjusting the treatment plan as necessary.
• Oversee discharge planning, providing appropriate follow-up care and ensuring that all patient interactions are thoroughly documented in an accurate and timely manner.

Patient Population / Client Focus

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution is required.

Compensation and Benefits
