Key Responsibilities

• Conduct thorough evaluations of patients, including reviewing medical histories, performing specialized assessments, and utilizing standardized evaluation tools to design effective treatment plans.
• Engage patients and their families in discussing evaluation results and treatment strategies, ensuring that goals are set collaboratively to enhance patient understanding and involvement.
• Implement individualized treatment protocols based on evaluation findings and continuously adapt the plan as patients progress or present with new challenges.
• Collaborate with a multidisciplinary team of healthcare professionals, maintaining clear communication regarding patient conditions and needed adjustments to the care plan.
• Manage discharge planning effectively, ensuring seamless transitions for patients to their next level of care with comprehensive follow-up strategies in place.
• Document all patient care activities accurately and in a timely manner, including detailed initial evaluations, progress notes, and discharge summaries to give a full picture of the patient's recovery journey.
• Prioritize excellent time management skills to meet productivity expectations while maintaining an organized schedule for patient visits and follow-ups.
• Participate in community and hospital-based marketing initiatives to promote our services and engage with the local population.
• Commit to ongoing professional development by pursuing continued education opportunities and serving as a mentor for students and new staff.

Patient Population / Client Focus

In this role, you will work with a diverse patient population experiencing a range of conditions, including musculoskeletal injuries, post-surgical rehabilitation, neurological impairments, and chronic pain. Your clinical expertise will play a crucial role in enhancing the outcomes of our patients, focusing on restoring mobility, improving strength, and minimizing pain.

Qualifications

• Minimum of a Bachelor's degree in Physical Therapy, with a preference for a Doctorate degree (DPT).
• A valid state license in Physical Therapy.
• Strong written and verbal communication skills, critical for patient interaction and documentation.
• Proficient in using electronic health record systems and basic computer applications for documentation and reporting purposes.
• A proactive approach to patient care, demonstrating flexibility and adaptability to meet the unique needs of each patient.

Preferred Experience

• A minimum of two years of professional experience in a physical therapy setting is strongly preferred.
