Clinical Supervisor: Nursing

Full-Time Clinical Supervisor: Nursing Coverage area: Sacramento This leadership role ensures patient care is coordinated and managed appropriately by a team of Registered Nurse (RN) Case Managers and LVNs. You will serve as the first point of escalation and support, participate in nursing education program development, evaluate program effectiveness, and maintain relationships with customers through reports and insights to improve outcomes and reduce readmissions. Field & Office Duties Hybrid work schedule: 2 days per week in the office for administrative and team management duties; 3 days per week (average 12+ per week) in the field conducting supervisory visits and providing direct support to RN Case Managers. Responsibilities Oversee clinicians in the field who provide care and treatment to patients at their residences. Handle incoming patient calls promptly and follow up to ensure appropriate actions are taken. Coordinate training and educational programs for field nurses to improve clinical outcomes and competencies. Assess staff learning needs, formulate specific plans and goals for each evaluation period, and serve as a role model and resource. Act as the first point of contact for field clinicians to assist in patient care escalations. Coordinate at least bi‑weekly case conferences with the clinical team to review patient progress and revise care plans as needed. Review and evaluate cases by assessing services provided, reviewing records, and providing guidance to clinicians to improve performance and quality of care. Review patient clinical diagnoses, medications, procedures, and clinical course. Assist clinicians in establishing immediate and long‑term therapeutic goals, setting priorities, and developing plans of care. Provide regular reports and updates to management, including readmission reports and insights for continuous improvement. Qualifications Two years of management or supervisory experience within the past five (5) years in a health‑care setting, preferably in Home Health. Graduate of an accredited school of nursing; Bachelor’s Degree preferred. Current California Registered Nurse (RN) license. Current Basic Life Support (BLS) certification. Valid California driver’s license with an acceptable driving record; all licenses and certifications must be current at hire and sustained throughout employment. Benefits 401(k) with matching.
