RN Care Manager - Ambulatory Care Management, Hybrid-Remote

Description As an RN Care Manager, you will function as a clinician, advocate, and educator to achieve optimal clinical and quality outcomes by effectively managing care and resources to reduce unnecessary utilization. The RN Care Manager utilizes clinical expertise and understanding of care management, CMS and NCQA regulatory requirements, quality measures of care, and provides safe and appropriate patient transitions across different levels of care. The RN Care Manager develops person-centered care goals, identifies barriers to health and implements interventions to meet patient goals through development of an individualized care plan with emphasis on promoting patient self -management and family responsibility. The RN Care Manager identifies and ranks relative importance and urgency of potential problems and plans contingencies. Accepts ownership of problems and willingly engages in the problem solving process, as an individual and as a group member. They maintain a positive attitude, when dealing with adversity and challenging situations and adapts to change effectively.
