Hospice Intake Coordinator

The Hospice Intake Coordinator manages the referral and admission process for patients entering hospice services. This role works closely with patients, families, referral sources, clinical staff, and leadership to ensure a smooth, timely transition into hospice care while delivering exceptional customer service and support.

Schedule?

Schedule: Full-time, in-office, 8:00 AM to 5:00 PM, with participation in a rotating weekend and holiday schedule.

Salary Range: $40,000 - $68,000 / Year

A brief overview of Responsibilities:

• Coordinates hospice referrals and admissions, verifies patient eligibility and insurance coverage, communicates with patients, families, and referral sources, and ensures all required documentation is completed for timely admission.
• Completes daily follow-up on pending referrals and provides admission status updates to leadership.
• Other duties as assigned.
