What You’ll Do
\nDeliver Patient-Centered Care
\n- \n
- Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential \n
- Develop and implement individualized care plans in collaboration with physicians and the care team \n
- Provide hands-on therapy to improve mobility, strength, balance, and independence \n
- Educate patients and caregivers on treatment plans, exercises, and safety strategies \n
Drive Outcomes & Recovery
\n- \n
- Monitor patient progress and adjust treatment plans as needed \n
- Support safe transitions of care and contribute to discharge planning \n
- Identify additional patient needs and collaborate with interdisciplinary teams \n
Collaborate & Communicate
\n- \n
- Partner closely with nurses, therapists, and physicians to ensure coordinated care \n
- Document patient care and progress accurately and timely within the EMR \n
- Participate in case conferences and team meetings \n
Lead & Support
\n- \n
- Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines \n
- Provide guidance and education to caregivers and support staff \n
\n
What You Bring
\n- \n
- Graduate of an accredited Physical Therapy program (DPT or Master’s) \n
- Current PT license in the state of practice \n
- Minimum 1–2 years of experience (home health experience preferred) \n
- Strong clinical assessment and critical thinking skills \n
- Excellent communication and patient education abilities \n
- Current CPR certification \n
- Valid driver’s license, reliable transportation, and auto insurance \n
