Key Responsibilities

• Conduct comprehensive evaluations of residents' physical capabilities and limitations to identify specific needs and challenges.
• Create, implement, and continuously modify individualized treatment plans designed to meet each patient's functional goals and maximize rehabilitation outcomes.
• Provide hands-on therapy services including therapeutic exercises, mobility training, pain management techniques, and patient education to promote healing and prevent future injuries.
• Continuously monitor and document each patient's progress towards their treatment goals, making necessary adjustments to therapy plans as conditions evolve.
• Maintain accurate and timely documentation using the facility's electronic health record (EHR) systems, ensuring compliance with all regulatory standards and internal policies.
• Collaborate closely with Physical Therapy Assistants (PTAs), Occupational Therapists (OTs), Speech Language Pathologists (SLPs), nurses, and physicians to ensure a coordinated approach to patient care.
• Educate patients, their families, and caregivers on exercises, safety protocols, and at-home care techniques, empowering them with the knowledge to support their ongoing recovery.

Patient Population / Client Focus

In this role, you will primarily work with diverse populations, including post-operative orthopedic patients, those with sports-related injuries, and individuals recovering from various functional impairments. Qualifications

• An active New York State Physical Therapist license is required.
• A graduate of an accredited Physical Therapy program (Doctor of Physical Therapy (DPT), Master of Physical Therapy (MPT), or equivalent).
• Current CPR/BLS certification to ensure accountability in emergency situations.
• Proven communication and clinical decision-making skills, along with a compassionate demeanor geared towards resident-centered care.
• Ability to navigate a fast-paced outpatient environment while maintaining a focus on high-quality patient care.

Preferred Experience

• Previous experience within a Skilled Nursing Facility (SNF) or a post-acute rehabilitation setting is preferred, as it provides valuable insight into the patient journey.
• Familiarity with Medicare guidelines and Patient Driven Payment Model (PDPM) documentation is an asset.
• Proficiency with electronic documentation systems (such as PointClickCare, NetHealth, or Rehab Optima) to streamline administrative tasks.
• Experience in specialized therapy areas including balance training, fall prevention strategies, orthopedic rehabilitation, and neurorehabilitation enhances the quality of care provided.

Compensation and Benefits

Pillar Orthopedics & Sports Medicine values the contributions of our therapists and offers a competitive salary range of $110,000 - $125,000 per year, commensurate with experience and qualifications. Additional benefits include:
