Key Responsibilities\n

As a Licensed Clinical Social Worker at Radiant Path, your responsibilities will include:

• Conducting thorough clinical assessments for patients enrolled in hospice care to determine their mental health needs, concerns, and preferences.

• Developing individualized treatment plans grounded in evidence-based practices, such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT).

• Providing psychotherapy services, including individual counseling sessions, group therapy, and family consultations focused on grief, loss, and adjustment to end-of-life transitions.

• Maintaining accurate and up-to-date clinical records in compliance with HIPAA regulations, ensuring confidentiality and best practices in documentation.

• Collaborating closely with a multidisciplinary team of healthcare providers—physicians, nurses, and spiritual advisors—to ensure holistic care for each patient.

• Facilitating regular case consultations and participating in peer review meetings to enhance the quality of therapeutic interventions.

• Educating patients and their families about mental health resources, coping mechanisms, and available community support services.

• Staying abreast of current research advancements in mental health and incorporating new methodologies into practice.

• Managing a diverse caseload effectively to ensure that patients receive timely and impactful care.

Patient Population / Client Focus\n

Your work will primarily focus on patients in hospice care as they navigate complex emotional landscapes. You will provide crucial support to not only individuals facing terminal diagnoses but also their families, addressing issues such as anticipatory grief, adjustment disorders, and existential questions about life and death. Qualifications\n

• A Master’s degree in Social Work from an accredited university.

• A valid license to practice as a Licensed Clinical Social Worker (LCSW) in New York.
