Job Description

Continuum is adding remote, nationwide positions for Social Work Care Navigators that will be responsible for resource referral, counseling, and coordination of care transitions for older adults with dementia and their caregivers.

Position: Care Navigator

Hours: Flexible to your schedule

We want you to join our team as our Care Navigator because you:

Communicate well with clients, families, healthcare providers, and resources
Coordinate transitions of care
Identify resources that could enhance independence and client/caregiver support
Understand dementia and its challenges for clients and caregivers
Gain rapport easily with others
Enjoy working with older adults and their families
Effectively manage your time to meet the position’s needs

How do you become our next Care Navigator

Hold a BSW or MSW degree or equivalent
Have experience working with older adults and memory impairment
Pass a background check
Have a positive, engaging, proactive approach
