Clinic Care Coordinator

Good Samaritan Health Center in Atlanta seeks a Clinic Care Coordinator-RN to lead care coordination for underserved patients in our faith-based, community clinic. The RN will provide triage, assessments, and chronic disease management; develop and monitor individualized care plans; and ensure seamless transitions across primary care, dental, behavioral health, and wellness services. Working closely with an interdisciplinary team, this role improves access, closes care gaps, and advances health equity while supporting patients with compassionate, whole-person, mission-driven care.
