Key Responsibilities\n

• Conduct individual therapeutic sessions with clients, utilizing a variety of evidence-based practices tailored to each patient's unique needs.

• Collaborate with prescribing clinicians to align your therapeutic services with the medication management plans for clients, ensuring a comprehensive treatment approach.

• Support discharge planning by facilitating advance care planning discussions that empower patients and their families to make informed decisions about their care.

• Document all patient care activities in our institutional electronic healthcare record, adhering to the specified documentation policies while ensuring that records reflect the quality of care delivered.

Patient Population / Client Focus\n

Your primary focus will be on patients receiving hospice care, a population that often requires a nuanced and sensitive approach to mental health. You will be tasked with addressing the psychological and emotional concerns that arise during end-of-life care, providing support to patients as well as their families. Qualifications\n

• Must possess a Master’s degree in Social Work, Counseling, Psychology, or a related field.

• Hold a current California state license as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n

This position offers an attractive salary range of USD 120,000 – 130,000 per year, commensurate with your experience and qualifications.
