Key Responsibilities

• Conduct thorough evaluations of patients in a home care setting to determine their physical therapy needs.
• Develop individualized treatment plans that align with therapeutic goals, using a diverse range of interventions such as exercise therapy, manual therapy techniques, and state-of-the-art rehabilitation equipment.
• Document patient treatments, responses, and progress meticulously to ensure continuity of care and regulatory compliance.
• Regularly reassess patients to adjust treatment plans as necessary, ensuring that their rehabilitation process remains effective.
• Instruct patients and their families on the use of assistive devices and effective self-care strategies, promoting independence and long-term well-being.

Patient Population / Client Focus

Qualifications

• Minimum of a Doctorate degree in Physical Therapy (DPT) from an accredited institution.
