Key Responsibilities

As a Physical Therapist at Confluent Rehabilitation, you will be responsible for:

• Directing and implementing physical therapy treatment plans that focus on preventing disabilities, alleviating pain, and restoring functionality in patients.
• Monitoring and overseeing the quality of care delivered by Physical Therapy Assistants, ensuring adherence to the established plans of care.
• Conducting thorough evaluations of patients' physical capabilities, utilizing diagnostic techniques to gauge muscle, nerve, and joint functionality.
• Collaborating with interdisciplinary team members to engage in case conferences and discussions to tailor comprehensive treatment approaches for our clients.
• Identifying potential enhancements within the home environment that facilitate rehabilitation and advising clients and caregivers accordingly.
• Documenting patient progress meticulously, including initial assessments, care plans, and discharge summaries, while ensuring compliance with OASIS and documentation standards.
• Providing patient education regarding therapeutic exercises, device usage, and the overall rehabilitation process.
• Engaging in persistent communication with physicians and case managers regarding patient responses to therapy and possible adjustments to care plans.
• Participating in continuing education and quality improvement initiatives to uphold the highest standards of patient care.

Patient Population / Client Focus

Our patient demographic includes individuals recovering from injuries, surgeries, and chronic conditions looking to regain independence in their daily activities. The Culpeper facility serves a diverse community, allowing our Physical Therapists to address a wide range of rehabilitation needs while also focusing on tailored solutions that meet individual patient requirements.

Qualifications

The ideal candidate should possess the following credentials:

• A Bachelor's Degree in Physical Therapy (Doctorate in Physical Therapy preferred).
• A valid and current Physical Therapy license in the state of Virginia.
• At least 2 years of clinical experience in an acute care or home care setting.
• Current CPR certification.
• A valid driver's license and automobile insurance.
• Strong interpersonal and communication skills to foster effective patient and family engagement.

Preferred Experience

Preference will be given to candidates with:

• Experience in rehabilitation settings that involve complex patient care.
• Proficiency in using various therapeutic modalities and rehabilitation technologies.

Compensation and Benefits
