Key Responsibilities

Your primary responsibilities will include:

• Collaborating with the therapy and facility team to assess patient needs and development of comprehensive treatment plans that facilitate optimal recovery.
• Educating patients and their families about therapeutic strategies, fostering greater engagement and understanding of the rehabilitation process.
• Conducting in-service training for facility staff to promote awareness of physical therapy services.
• Participating in Care Plan Meetings to share insights and detailed progress reports on patient rehabilitation journeys.
• Maintaining accurate and timely documentation that adheres to regulatory standards and best practices.
• Working closely with the supervising Physical Therapist to ensure continuity of care throughout the rehabilitation process.

Patient Population / Client Focus

This position focuses on a diverse patient population, including individuals recovering from fractures, surgeries, and other musculoskeletal injuries. Qualifications

To qualify for this position, candidates should possess:

• A valid Physical Therapy license in good standing in Virginia.
• A Bachelor's Degree in Physical Therapy; a Doctorate in Physical Therapy is preferred.
• Current CPR certification.
• A valid driver's license and reliable transportation for home care visits.
• A commitment to clinical excellence and a strong work ethic as well as a collaborative spirit.
• Outstanding communication skills, enabling you to effectively engage with patients, families, and colleagues.
