Key Responsibilities\n

• Lead and facilitate therapy groups focused on psychoeducation and processing individual experiences, promoting resilience and coping strategies among participants.

• Perform clinical assessments to identify patient needs, develop treatment plans, and monitor progress throughout their treatment journey.

• Collaborate with the Utilization Review Department to secure authorization for services, manage billing, and ensure timely payment processes.

• Initiate discharge planning for all patients upon admission, facilitating a smooth transition and continuity of care.

• Complete discharge sessions to ensure clients understand coping mechanisms, transition plans, and community resources available post-treatment.
\n
• Ensure all follow-up appointments are confirmed with patients and family members prior to discharge to promote ongoing care.
\n
• Engage with referral sources and outpatient providers throughout the admission and discharge processes to guarantee seamless continuity of care.

Patient Population / Client Focus\n

In this role, you will work with a diverse demographic of clients seeking mental health support, from children and adolescents to adults facing various mental health challenges. You will help address issues such as anxiety, depression, trauma, and relationship difficulties within a community setting, covering aspects of both individual and group therapy.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Must hold a valid, active Clinical Social Worker (LCSW) license in the state of California.

• Exceptional written and verbal communication skills are essential for effective interaction with patients, families, and multidisciplinary team members.
