Description

• Conduct assessments for patients in inpatient and outpatient environments, crafting tailored treatment plans for diverse needs
• Implement and oversee physical therapy modalities focused on enhancing mobility, strength, balance, and overall functional capacity
• Engage in interdisciplinary collaboration with nurses, physicians, case managers, and rehabilitation specialists to best support our clients' health
• Provide education to patients and their families on therapeutic intentions, plans for discharge, and home exercise regimens
• Maintain detailed documentation of assessments, treatments, and progress per facility standards and regulatory requirements
• Participate in interdisciplinary rounds and discussions for seamless patient-centered care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent program from an accredited institution

Certifications

• Active state licensure as a Physical Therapist
• Current CPR certification is preferred
• Any specialized certifications (e.g., geriatrics, neurology) will be considered advantageous but are not mandatory

• Prior experience as a physical therapist is highly preferred
