Key Responsibilities\n

As a Licensed Clinical Social Worker in our hospice center, your responsibilities will encompass a variety of crucial tasks:

• Collaboratively developing personalized treatment plans that incorporate evidence-based interventions such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT), ensuring alignment with the patient’s treatment preferences.

• Providing trauma-informed psychotherapy services tailored to individuals coping with life-limiting illnesses, focusing on alleviating symptoms of anxiety, depression, and grief.

• Documenting all client interactions and treatment progress accurately and promptly, maintaining compliance with HIPAA regulations and best practices.

• participating actively in interdisciplinary team meetings to coordinate care strategies and interventions with other healthcare team members, enhancing our approach to patient care.

Patient Population / Client Focus\n

This position focuses primarily on adult patients within a hospice setting, providing psychological support during critical life transitions. You will collaborate with patients experiencing advanced medical conditions, facilitating their navigation through emotional, psychological, and social challenges, while also supporting families through the grieving process.

Qualifications\n

• A Master's degree in Social Work (MSW) from an accredited institution is essential.

• A valid license as a Licensed Clinical Social Worker (LCSW) in the state of Georgia.
