DENTAL-SCHOOL BASED SEALANT PROGRAM
INTERNAL AGENCY OPPORTUNITY
This position requires you to work on site at one of our facilities located throughout Broward County.
This position requires in-office work.

POSITION DUTIES & RESPONSIBILITIES:
This position is responsible for the review of consent forms from clients for the School Based Sealant Program. The position requires maintenance of unique records, files, reports, patient records, and related correspondence to schools or parents. This position requires the employee to be detail oriented and notate any missing information. The employee must feel comfortable with calling the school contact or the parent to retrieve missing information in a professional manner.
- Serves as the screener for consent forms for clients.
- Keep all consent forms, packets, necessary forms organized in alphabetical order by school and grade.
- Review the necessary documents for a child to participate in the program.
- If any documentation is missing, the employee must feel comfortable to call and speak with school contact or parent of child to retrieve missing information.
- Assist parents or school contacts with any general questions they may have regarding the program.
- Use HMS database to cross reference any missing information from the consent forms.
- Other duties as assigned.

MINIMUM QUALIFICATIONS:
- Current employee of Florida Department of Health. Applicants accepted only from Department of Health employees, (includes OPS, Career Service, SES or SMS employees). Applicants will not be accepted from outside of the department.
- Possess a valid driver's license, free of major infractions and access to an automobile to be utilized for work purposes and willing to drive to multiple sites as required by program needs.
- Two (2) or more years of professional experience with customer service in a dental or medical office setting.
- Two (2) or more years of professional experience with strong communication skills making calls to clients.
- Two (2) or more years of professional experience working in a School-Based dental program screening consent forms.
- Two (2) or more years of professional experience handling confidential patient information.
PREFERRED QUALIFICATIONS:
- Associate's degree or higher from an accredited college or university.
- Bilingual, fluent in English/Spanish or English/Creole.
