Work Environment

This position is based onsite, where you will operate primarily from patients' homes. You'll be part of a supportive, collaborative team consisting of two other physical therapists and a physical therapy assistant (PTA), who share a common goal of providing exceptional care to our patients. Key Responsibilities

• Conduct thorough assessments to understand each patient's goals and needs, and develop tailored care plans that highlight effective treatment interventions.
• Manage a caseload where you will work one-on-one with patients for an hour, ensuring personalized attention throughout their rehabilitation journey.
• Utilize a variety of evidence-based treatment techniques and modalities aimed at improving mobility, strength, and overall patient wellness.
• Educate patients on exercises and activities designed to facilitate recovery, minimize reliance on medications, and promote long-term health.
• Collaborate closely with other healthcare professionals to ensure a cohesive approach to patient care, sharing insights and updates on patients' progress.
• Continuously evaluate treatment effectiveness, adjusting care plans as needed to optimize patient outcomes.
• Document and record all patient services comprehensively and accurately, ensuring compliance with organizational standards.
• Motivate and encourage patients to adhere to their treatment plans and foster a proactive approach to their rehabilitation.

Patient Population / Client Focus

This position focuses primarily on patients requiring orthopedic rehabilitation, targeting recovery from injuries, surgeries, and chronic pain conditions. Our patient demographic typically comprises individuals with private medical insurance, reflecting a diverse range of ages and health backgrounds. You will have the privilege to guide these individuals in reclaiming their physical capabilities and improving their quality of life.

Qualifications

• Possession of a Bachelor's Degree in Physical Therapy from an accredited program.
• Minimum of 2 years of experience working as a Physical Therapist, ideally in an orthopedic outpatient setting.
• Active Physical Therapist license in New York with no disciplinary actions on record.
• Demonstrated proficiency in manual therapy techniques and exercise program implementation.
• Familiarity with Electronic Medical Record (EMR) Software, specifically WebPT, will be advantageous.

Preferred Experience

Experience in home care settings and a proven track record of effectively developing customized treatment plans that yield positive patient outcomes will make you an ideal candidate for this role. Compensation and Benefits

Your annual salary for this position will range between USD 100,000 and USD 120,000, commensurate with experience and qualifications.
